APPLICATION INSTRUCTIONS

Please note that the following instructions are mandatory. The application will be considered ONLY if all directions are followed. You will be required to apply again if the application is incomplete.

1. All names on the application MUST match names on the passport (word for word).

2. The application should only include immediate family members, which includes a husband, wife and children. This does NOT include sisters, brothers, mothers and fathers. If you include anyone other than immediate family, they have to be listed under the "Comments" section.

3. The "References" and "Summary of your background" sections MUST be completed. The "Summary of your background" section has to be detailed with your work and personal background (must be at least four sentences). You must include at least two different references.

4. All birthdates must follow the format below:

   • dd/mm/yyyy (e.g. 27/10/1942)
